Awards Nomination

Do you know an individual or team doing remarkable work in surgical quality improvement?

Nominate them!

The award categories for the Ontario Surgical Quality Conference 2018 include:

  • Greatest Improvement in Post-Surgical Outcomes: Given to the surgical team who has demonstrated the greatest improvement in postoperative outcomes
  • Most Engaged Surgical Clinical Reviewer: Given to the Surgical Clinical Reviewer who has displayed exceptional leadership through their involvement in the surgical network community of practice. This includes participation in monthly calls, Quorum, workshops and conferences
  • Most Engaged Surgeon Champion: Given to the Surgeon Champion who has displayed exceptional leadership through their involvement in the surgical network community of practice. Including participating in monthly calls and presenting at conferences
  • Innovation Award: Given to the surgical team who has successfully implemented innovative change ideas that resulted in improved surgical outcomes
  • Excellence in QI Implementation: Given to the team who has displayed a commitment to quality improvement methodology in the pursuit of better surgical outcomes
  • Interprofessional Team Award: Given to the surgical team who has assembled professionally diverse group that includes not only the surgeons and Surgical Clinical Reviewers, but also representation from hospital leaders and frontline staff
  • Most Valuable Player: Given to the surgical network member who has displayed an outstanding commitment to surgical quality improvement

Nomination submissions close Friday, October 19, 2018.

Awards will be presented live at the Ontario Surgical Quality Conference at 3:30 pm in the Ballroom at the Chestnut Residence & Conference Centre.
